Your policy may require a referral to physical therapy by your primary care physician. There may be limitations regarding the number of visits you can receive. Please contact our billing department if you have questions regarding your insurance.
IF YOU ARE USING A CREDIT CARD for payment of our services – there will be a 3.5% processing fee added to your total for each credit card transaction.
